Hepatitis crónica por VHC en la población usuaria de drogas por vía parenteral. Motivos para no iniciar el tratamiento antiviral

Resumen


Objetivos: Analizar la prevalencia de usuarios de drogas por vía parenteral con una hepatitis crónica relacionada con el VHC que no reciben tratamiento antiviral y las razones del mismo. Pacientes: 1269 pacientes con una infección por el VHC fueron remitidos para evaluación terapéutica entre enero de 1990 y enero de 2000. 416 (32,7%)fueron usuarios de drogas por vía parenteral (UDVP). Métodos: Se analizaron las variables demográficas, clínicas, bilógicas e histológicas de forma prospectiva. Los pacientes UDVP fueron divididos en dos grupos: tratados y no tratados. Resultados: En el grupo de pacientes UDVP, 268 fueron tratados y en 148 casos (35,5%) no se consideró indicado el tratamiento antiviral. Las causas más frecuentes para no iniciar el tratamiento antiviral fueron la presencia de una toxicomanía activa, la negativa del paciente a recibir dicho tratamiento y la presencia de patología concomitante severa. Conclusiones: Un amplio subgrupo de pacientes UDVP con una hepatopatía relacionada con el VHC no reciben tratamiento antiviral.
