La CIE debería de eliminar la especificación etiológica en la mayoría de diagnósticos atribuibles al alcohol

Resumen


El uso de alcohol es un factor de riesgo para muchas enfermedades crónicas, de las cuales más de 40 son atribuibles al alcohol. El hecho de especificar la etiología en los nombres de las enfermedades atribuibles al alcohol en la Clasificación Internacional de Enfermedades (CIE) ha resultado en que las personas afectadas sean objetos de estigma, diagnósticos erróneos y tratamientos inadecuados. Otros efectos de esta práctica también pueden incluir un atraso de la atención sanitaria e inexactitud de los informes. Usamos dos ejemplos para debatir las consecuencias de especificar «alcohol» en los nombres de las enfermedades con una etiología vinculada al alcohol: la hepatopatía alcohólica y el síndrome del espectro alcohólico fetal (SAF). Respecto de su sintomatología y tratamiento, cada una de estas condiciones no es única ni se diferencia de sus homólogos: las enfermedades hepáticas no relacionadas con el alcohol y los trastornos del neurodesarrollo idiopáticos. La especificación de «alcohol» en el nombre de una enfermedad tiene varias consecuencias negativas pero ningún beneficio claro. Dado que la CIE se usa como el estándar internacional para informar sobre las enfermedades y las condiciones de salud, la existencia de categorías de salud que inhiban la capacidad de los profesionales sanitarios para reportar enfermedades de manera fiable es contraproducente.
